Abstract
Highlighting the transformative role of digital technologies in advancing population health, this track will explore how artificial intelligence (AI) can enhance risk stratification, care navigation, predictive modelling, and personalised health interventions at scale. Sessions will also examine the development of interoperable data platforms that integrate clinical, community, and social data to support coordinated, data-driven care. Complementing these foundational concepts, the track will feature local use cases that demonstrate how digital tools are already improving outcomes, streamlining workflows, and enabling more proactive and efficient population-level interventions.
