Abstract
This track focuses on the design and delivery of integrated care models that provide seamless, coordinated support for individuals as they move across services, care settings and life stages. It will examine transitional care for children and adolescents entering adult services, highlighting the clinical, social, and developmental considerations essential for continuity and long-term wellbeing. The track will also explore integrated approaches for people living with chronic conditions and dementia, showcasing models that bring together medical, community, and caregiver support to reduce fragmentation and improve quality of life. In addition, it will address justice health by examining reintegration pathways for individuals transitioning from prison back into the community — a critical but often overlooked area of population health. Together, these sessions illustrate how integrated care models can bridge gaps, reduce inequalities, and support more person-centred, coordinated health journeys. term wellbeing.
