Abstract

This track examines care models that bring health services closer to where people live, recognising the home and community as vital settings for preventive, restorative, and long term care. It will explore how patient centric home based care can strengthen continuity, reduce avoidable hospital use, and support ageing in place. Sessions will highlight community care innovations, including integrated neighbourhood based services and mobile community hospital models that extend subacute and rehabilitative care into patients’ own environments. The track will also feature perspectives on home based palliative care, showcasing approaches that enable dignified, personalised support at the end of life. Additionally, it will spotlight emerging modalities such as tele dentistry, illustrating how digital tools can expand access and bridge service gaps. Collectively, this track showcases practical, scalable ways to shift care upstream and into the home and community, enhancing accessibility, person centredness, and overall system resilience.
