Successful population health efforts leave no one behind. Yet within every community, there are groups whose circumstances, environments, or life experiences result in distinct health needs, care challenges, and outcomes that differ from those of the broader population. Understanding and responding to these differences is essential to building a more equitable and inclusive health system.

Today, we bring together leaders working with three special populations: nursing home residents, incarcerated persons, and migrant workers. Drawing on their experiences across aged care, correctional health, and migrant worker health, the speakers will share insights into the populations they serve, the programmes and initiatives developed with and for them, and the factors that shape their health and wellbeing.

Through these complementary perspectives, the discussion will explore the unique challenges, considerations, and approaches involved in delivering care to populations whose needs may not always be adequately addressed through mainstream models of care. The speakers will also reflect on how health systems, multidisciplinary teams, and community partnerships can work together to improve access, experiences, and outcomes for these groups.

Participants will gain a deeper understanding of the diverse needs of special populations and learn how person-centred, equitable, and responsive approaches can contribute to better health outcomes for all members of the community.
