Abstract

This track explores the evolving population health workforce and the emerging roles essential for delivering integrated, community based care. It will highlight the concept of the “third” workforce—comprising non traditional, community embedded roles such as Well-Being Coordinators (WBCs)—and how these new capacities are reshaping care delivery outside conventional clinical settings. Sessions will examine the growing contributions of health coaches across different clusters, including their scope, competencies, and impact on behaviour change, chronic disease management, and preventive care. The track will also focus on the expanding autonomy and scope of community nurses, showcasing how advanced practice, home visit models, and greater decision making authority enable more proactive, person centred support. Together, this track provides a forward looking view of how multidisciplinary workforce models can strengthen population health and meet the rising demand for community based care.
