Ms Stephanie Teo served as Chief Nurse of SingHealth Polyclinics from October 2015 to September 2024, providing strategic leadership in nursing and patient care services, with a strong focus on clinical excellence, education, innovation, and applied research. A community nursing-trained leader, she was appointed Director of Community Nursing, SingHealth, in 2020, where she played a key role in shaping policies, frameworks, and care models to strengthen integrated and continuing care across eastern Singapore. Her work centred on advancing preventive health, empowering patients in self-care, managing complex health needs, driving innovation and research, and standardising community nursing practices to support population health and build a future-ready nursing workforce.

Currently, as Director, Special Projects, Group Nursing, she provides strategic leadership, project oversight, and cross-institutional coordination of nursing initiatives across SingHealth. She also drives care integration with community and healthcare partners to support the delivery of safe, high-quality patient care, while mentoring and developing future nursing leaders.
